 iSCSI Authorization Failure Cases

Case 1: After upgrading EqualLogic SAN firmware, some of our servers start to get "Authorization Failure". I fixed the problem after I read this How to enable simultaneous connections from imitators with different IQN names article.

Case 2: In my case, I entered incorrect CHAP username.

Case 3: In other case, the client enter incorrect iSCSI initiator name. Normally, we recommend our client copying the initiator name instead of typing it.
