Can't start VM with Event ID 12010 and 12240

Situation: the client is running Windows 2008 R2 Hyper-V. They can't start the VM with these event errors:

Case 1: The iSCSI Initiator service is not running. Restarting it fixes the problem.

Case 2: The iSCSI Initiator lost SAN drive. Re-adding it fixes the problem.

Case 3: failed to login The iSCSI Initiator.
